Adherence with standard guidelines among doctors involved in aerosol generating procedures to control COVID-19 transmission

Objectives: To review the strategies for controlling COVID-19 transmission among doctors involved in aerosol generating procedures. Methods: A cross-sectional study was conducted in Pakistan Institute of Medical Sciences, Islamabad from September to November 2020, using a 21-itemed initially piloted self-administered questionnaire designed on Google forms. It was shared with doctors involved in Aerosol generating medical procedures (AGMPs) by using social media applications. After the data was collected, it was analyzed using SPSS version 21. Results: A total of 136 doctors participated in the study that included 59 female and 77 male participants. Our 67 (49.3%) participants screened their patients for signs and symptoms of COVID-19 before a routine checkup. The most common disinfectant used for reusable medical equipment by 62 (45.6%) respondents was alcohol swab, 57 (41.9%) did not disinfect reusable equipment and 17 (12.5%) used Dettol water spray. Epidemiological trends in community acquired acute Kidney Injury in Pakistan: 25 years Experiencefrom a Tertiary Care Renal Unit

Background: Epidemiological studies of community acquired acute kidney injury (AKI) are sparse especially from South Asia and none has published from Pakistan. Reported incidences from different countries vary with use of different criteria of defining AKI. There is also variation found in different class of income countries, hospital based versus community based AKI. Methods: The current study was carried out in all adult AKI patients developing community acquired AKI and coming to a tertiary care renal institution from January 1990 to December 2014. This is a retrospective data collection from patient’s records and AKI was defined according to KDIGO guidelines. Trends among different groups which are classified in medical, obstetrical and surgical were observed and presented. Comparison of graft uptake in patients undergoing myringoplasty/ tympanoplasty by overlay and underlay procedure

Objectives: To determine which method underlay or overlay has higher results in graft uptake in Tympanoplasty. Study Design: It was experimental study. Setting: it was carried out at ENT Department of Benazir Bhutto Hospital, Rawalpindi. Period: One year from March 2017 to February 2018. Material and Methods: It consisted of a total of 27 patients presenting with chronic otitis media with dry perforation. Two groups of patients were made. Group A had 14 members who undergone underlay technique while the 13 patients in group B undergone overlay technique. Comparison was made on basis of successful graft uptake the main criteria of which was absence of lateralization, atelectasis, blunting or formation of retraction pockets by the end of 6 weeks of follow up. Dengue in the urban slums of Pakistan: health costs, adaptation practices, and the role of dengue-diagnosis and surveillance in controlling the epidemic

Dengue virus (DENV) infection is a public health issue in tropical and subtropical climates, including in Pakistan. A significant population in Pakistan lives in urban slums, where residents are highly susceptible to the risk of DENV transmission. We examine health costs of DENV infection, determinants of adaptation practices and the role of surveillance to control DENV infection using cross-sectional data from urban slums of Pakistan. Information from respondents with and without dengue history indicated that slum dwellers have very little knowledge of dengue mosquitoes and viral transmissions. Practices adopted by the slum dwellers to avoid DENV infection risk appeared to be unsatisfactory. We employed a health belief model to examine factors affecting control practices. Participation in dengue awareness sessions, information from media, and dengue-diagnosis and surveillance team visits are found to be important determinants of high adaptation practices to avoid DENV infection. Dengue surveillance system for controlling DENV infection in urban Pakistani slums needs improvements. Rigorous and continuous monitoring should be vigilantly performed in areas where DENV infection cases originate. Regular presentation of dengue awareness sessions at a union council level and providing timely and appropriate healthcare education to the slum communities could improve household adaptation practices and lower DENV infection rates.
Medical Nutrition Therapy in Renal Replacement Therapy

This chapter discusses Medical Nutrition Therapy in three modalities of renal replacement therapy (RRT), outlining the nutrient requirements for patients on maintenance hemodialysis, peritoneal dialysis, and kidney transplant in acute and chronic phases. The chapter takes note of the drastic impact of RRT on the patient's nutrition status and overall well-being, which puts them at high risk of morbidity and mortality, and thus emphasizes timely and regular comprehensive nutrition assessment to enable appropriate intervention. Recognizing that there are different modalities of RRT and that patients have different physiological characteristics as well as different laboratory test values, which may also vary for individual patients each time tests are run, nutrition therapy is individualized each time. The chapter takes a closer look at protein-energy wasting, a condition common among patients undergoing RRT, which is a predictor of mortality, discussing its prevention and treatment measures. Finally, the chapter takes a closer look at electrolytes, specifically potassium, sodium, calcium, and phosphorous, in relation to mineral bone disease.
